FormBase: Forms for user data submission
Forms can be linked to: public or private webpages, calendar items, news items or emails. The form can contain any number of questions, grouped in sections and can include multiple choice, text, check boxes, etc. Branding and graphics can be added to the form using the standard Axis editor. The confirmation page can also contain graphics created using the standard Axis editor, as can a confirmation email sent to anyone submitting a form. One or more administrators can be notified of any submissions. Collected data, including fields that can only be seen by administrators, are stored in a database. Optionally, users can return to view their answers and change them. The FormBase databases include reporting, a report-writer and exporting to Excel . Those submitting a certain form type can be treated as a group and sent a mass email. |
Building Mini-Databases
Many clients need to build up mini-databases of information submitted by members or visitors online. They might want to capture information about those interested in using banquet facilities, or those who would volunteer to car-pool for the swim team, or those interested in membership. FormBase provides the tools to ask the questions and then save and report on the answers. |
